Cancellation Policy

We kindly ask that you cancel your appointment online or by phone more than 48 hours in advance to avoid the cancellation fee. Your signature on this form indicates your agreement to comply with this policy. Thank you for understanding and we look forward to helping you achieve your beauty goals.

Your appointment time is reserved just for you, and we look forward to your personalized treatment session at Haven. Our cancellation policy is no charge to the client if canceled prior to 48 hours of the scheduled appointment. If canceled within the 48-hour window of the scheduled appointment, the card on file will be charged the full cost of the service.

By signing this form, I give Haven Permanent Cosmetics and Aesthetics permission to charge my card on file to follow cancellation or no-show policy.

You acknowledge this is a nonrefundable service/charge. If a deposit is required, you understand this is also a nonrefundable deposit before or after the service for any reason. You agree you will not dispute payment with the credit card company or bank and agree that this is a binding contract upon your booking. You also agree there are no refunds for services before or after a service. All products are Nonrefundable and are final sale at Haven Permanent Cosmetics and Aesthetics. Products cannot be returned or traded out for any reason.
